3. List of major projects/activities for past 3 years (including this year);
4. List of current Officers and Members of the Board of Trustees;
5. List of current Core staff and;
6. Most recent Annual Report and Financial Statement.

Application Fee : P2,000.00 upon approval of membership

Annual Dues :

• P1,000.00 (if paid during the 1st quarter); and
• P1,500.00 (if paid after 1st quarter and not later than May of each year)
